Clinical presentation

A tender erythematous papule develops 4 to 7 days after contact and then evolves into a pustule. Pustules rupture after 2 to 3 days to form painful shallow ulcers with granulomatous bases and purulent exudates. The ulcer edge is ragged and undermined. Ulceration takes several weeks or months to resolve if not treated. Lesions occur on the prepuce and frenulum in men and on the vulva, cervix, and perianal area in women. Mixed infections with H ducreyi, Treponema pallidum, and HSV may occur. Painful, tender inguinal lymphadenitis occurs in up to 50% of cases and the lymph nodes may develop into buboes. The lymphadenopathy is usually unilateral and tends to be more prevalent in men. Buboes can rupture spontaneously.
