Lichen Sclerosus TreatmentAdvise gentle cleansing of involved areas using Cetaphil cleanser, wearing of ventilated cotton underwear and liberal use of bland emollients and lubricants. Avoid panty liners, pre-mixed douches and sprays, and scented toilet paper. Topical application of group V steroid creams for 2 weeks is initial treatment for uncomplicated skin lesions and prepubertal mucosal lesions. Clobetasol propionate 0.05% (is effective for vaginal and penile disease in adults. Intralesional triamcinolone is useful for unresponsive areas. Tacrolimus ointment (Protopic) 0.1% may be of help in treating vaginal or penile disease. Testosterone propionate 2% ointment has been used for years. Many experts now doubt its effectiveness. Topical 5-fluorouracil with carbon dioxide laser has been used for penile lesions with some success. Acitretin (20 to 30 mg/day for 16 weeks) may be considered in severe cases. Selected refractory medical cases may benefit from surgical reconstruction of scarred areas. |
