Lichen planus Treatment

There is no universally effective treatment for lichen planus. Topical and systemic corticosteroids are the mainstay of therapy. Prednisone for generalized skin or erosive mucosal involvement; a 4-week course starting at 1 mg/kg/day and gradually decrease of the dosage.

Group I or II topical steroids twice daily are used as initial topical treatment for localized disease. Intralesional triamcinolone acetonide is often useful for hypertrophic lesions. Tacrolimus (Protopic) 0.1% ointment twice daily has been used with some success for erosive oral lesions. Sedating antihistamines (hydroxyzine 10 to 25 mg every 4 hours ) for itching.

Retinoids (Acitretin) 1 mg/kg/day, cyclosporine (5 to 6 mg/kg/day) may be considered for severe recalcitrant forms of lichen planus.
