Discoid Lupus Erythematosus – Histology

DLE lesions demonstrate interface changes at the dermal-epidermal junction and the basement membrane zone region of follicular epithelium. Necrotic keratinocytes and lymphocytic inflammation are present in these interface areas. There is superficial and deep inflammation consisting of lymphocytes and plasma cells. The follicles are eventually replaced by fibrosis and there may be interfollicular dermal scarring. Increased dermal mucin is often present. Direct immunofluorescence may show linear granular deposits of C3 and IgG at the dermal-epidermal junction and in the basement membrane zone area of follicular infundibula.
